If your complaint has not been resolved to your satisfaction, and if you wish to file a formal complaint we require you to put your complaint in writing. 

The complaint officer will acknowledge receipt of your concern within two business days. They will provide you with a response immediately following the completion of their investigation.

Once this form is submitted the information will be sent directly to our Ombudsman. We will never use your email address for any other purpose.
